Supported Living Team Leader
• Salary: £22,000 - £24,000
• Location: North London
• Report to the Supported Living Manager.
Your purpose will be to enable people with mental health needs, disabilities and associated needs to improve and maintain the quality of their lives by:
• Support and supervision of staff and acting as Shift leader.
• Support in achieving daily living skills.
• Identifying and meeting the needs of Individuals
• Developing and maintaining relationships with Service Users, staff, and the local community
• To work additional hours when authorised and as necessitated by the needs of the business and take part in on-call.
Registered Learning Disability Nurse RNLD

The patient group consists of Personality Disordered Learning Disability and Enduring Mental Illness. As a Registered Nurse, you will provide direct care and interventions to Patient/Service Users without direct supervision and coordinate the care given by colleagues.

To function as a Care Coordinator for named clients and to assist in ensuring individuals’ needs are addressed proactively through sound care planning, including meeting clinical needs.
Lead the care team to ensure that residents’ physical, emotional and social needs are identified and met through assessment and care planning.
To assist the Area Manager by leading the care team in achieving and, where possible, exceeding care standards determined by CQC, Local Authority Contract Monitoring.
To encourage effective working relationships within the houses and with the multi-disciplinary team approach to promote a high-quality service.
Maintain close working relationships with professional partners to achieve successful clinical care outcomes and achievement of management performance objectives.
To promote a therapeutic environment in line with the principles of ordinary living.
To assist in the management of a defined staff team, including the supervision of staff and the leading of shifts on a day-to-day basis.
